What we do
Riverside offers a range of care and support services designed to help vulnerable people leave hospital sooner and recover in their own community. This includes care navigation and co-ordination, intermediate care accommodation, home preparation services, telehealth care monitoring and care at home. These service elements can be combined to tailor each service to the needs of commissioners and their communities.
