Social Prescribing is an approach that connects people to activities, groups and services in their community to meet the practical, social, and emotional needs that affect their health and wellbeing.
Social Prescribers, also known as Link Workers, listen to people, focusing on a ‘what matters to me’ approach to coproduce personalised support plans, supporting people to take control of their health and wellbeing.
Involve partner with 19 PCNs and provide over 50 staff through the Additional Role Reimbursement Scheme (ARRs).
