Caring for someone

Do you look after a partner, relative or friend who due to physical or mental illness, age related difficulties, disability or an addiction cannot manage without your support?

There are many different reasons why people may start caring for someone, and this can happen at any stage in their lives.

We understand you may not think of yourself as a Carer but if you are 16 and over and provide care and or support to an adult (18 and over) our service for Carers is for you.

How we can help

We can identify with you what services or support might be helpful to you and help you to access it. We can inform you of your rights as a Carer and any benefits you may be entitled to

We will meet with you to discuss what impact caring may have on your health and your life. We will identify with you what may be available to help you and develop a support plan with you to meet any eligible needs. We provide this service on behalf of Kent County Council. https://www.kent.gov.uk/__data/assets/pdf_file/0013/30145/carers-assessment.pdf

A dedicated service for Carers who are aged 16 to 24 with access to information, advice and guidance. 1 to 1 support to ensure you can live the life you wish to lead, access to support groups and activities to meet other young adult carers.

If you or the person you care for is admitted to Maidstone or Tunbridge Wells hospitals our dedicated Hospital Navigators are availableThey can support you to ensure a safe discharge and this may include brokered support to help you with your caring role on discharge from hospital.

Our Navigators offer a listening ear for you to talk about your caring role and concerns you may have.

You can meet other Carers at support groups, workshops, Carers Forums and activities (face to face and virtual). You can have access to the Kent Carer’s Emergency Card so that someone close to you can be contacted to support with your caring role in an emergency and we can help you to draw up an emergency plan for if you are unwell or in hospital.

“I didn’t have the courage to reach out before I had my Carer’s Assessment, but my Community Navigator didn’t pass judgement and we worked together to help me identify what I needed, one step at a time. I am so grateful for the support I received, I have my life back again. The difference she made is immeasurable.”
