Community Navigator - Telephone Team

Up to 24,000, Maidstone

Advising and supporting older people and Carers (those who care for a family member or friend) by telephone with information, advice, and support. Navigating them to appropriate information and support to improve their wellbeing. Completing wellbeing assessments by telephone and accessing appropriate support. Completing assessments to identify a Carer’s needs and developing support plans with them.

Social Prescribing Link Worker

Up to £23,000, Maidstone 

Working within GP practices to engage, refer and connect patients to non clinical services and community groups. 

Community Activity Coordinator

Up to £23,000, Tunbridge Wells and the Weald

Help connect isolated and vulnerable older people by organising interesting social, cultural, leisure and outdoors events, groups, and activities in the community where they live.

Social Prescribing Team Lead

Up to £25,500, Turkey Mill Maidstone and supporting team throughout Kent.

Leading a team of staff who work within GP practices to engage, refer, and connect patients to non-clinical services and community groups. 

 

Social Prescribing Link Worker – Long term health conditions x 2

Up to £23,460, Turkey Mill, Maidstone. Working throughout West Kent.

Working with GP practices and secondary healthcare services to engage,\refer\ and connect patients to non-clinical services and community groups.

Business Support Assistant – GP primary care network focus
Up to £19,000, Turkey Mill, Maidstone

The Business Support Assistant will take on a new role supporting the Senior Management Team, Business Manager and Involve Services including administrative duties, recruitment, data reporting and collation with a particular focus on our GP primary care network contracts.

Digital & IT Officer

£21,000 per annum (FTE), Maidstone

The suitable candidate will be tasked with assisting the Digital and IT Lead with a range of responsibilities which are detailed in the title link above. Usual placement will be at Turkey Mill with occasional visits to our Hub office with the option to work from home on some days. For the current period, home working can be carried out except for when responsibilities can only be carried out at the offices. The IT department maintains a strong presence and is responsible for ensuring that all services and their staff can function using technology.
