Vacancies:

Care Coordinator – Palliative Care

Up to £22,000 FTE, Sittingbourne GP Primary Care Network

To empower and support people with an end-of-life diagnosis, enabling them to navigate services and community support and take an active role in their end-of-life planning.

Children's Health and Wellbeing Navigator

£23,500 - £24,400 FTE, Medway Central (Chatham & Gillingham) GP primary care network

Improve support for children with mental health, neurodevelopmental and other health conditions, and their families, by navigating them through processes (eg diagnosis) services and structures and linking them to local groups and initiatives to improve wellbeing.

Community Navigator – (Older People and Carers)

£24,000 FTE, Office Turkey Mill, Maidstone and (ability to work from home too due to pandemic).

This is an innovative role working on a KCC commissioned service to develop the Community Navigation service for older people (over 55) and Carers (those caring for a family member or friend) across West Kent.

Helpline Officer- Vaccinations (Temporary)

£12-15 per hour, Full or Part Time between Mon-Fri 8.30 am – 5pm
Office Turkey Mill, Maidstone and (ability to work from home too due to pandemic).

This role involves working on a Kent and Medway NHS Clinical Commissioning Group funded pilot project, providing telephone-based information to members of the public who are hesitant about receiving the Covid-19 vaccine.  

Mental Health Care Navigator and Coordinator

£23,500 - £24,400 FTE, Medway Central (Chatham & Gillingham) GP primary care network

To empower and support people with mental health issues, enabling them to make the most of services and community support or similar.

Social Prescribing Link Worker

Up to £23,000 FTE, The Weald (Staplehurst, Marden, Cranbrook, Yalding and Hawkhurst) GP primary care network

Working within GP practices to engage, refer and connect patients to non clinical services and community groups.
