Support via your GP
We offer personalised, 1-to-1 support at over 40 GP surgeries across Kent. Our friendly Children’s Health and Wellbeing Navigators help support children and young people (aged up to 18 or 24 for those with SEND) with a long-term health condition, neurodevelopment or sensory disabilities (e.g. ASD and ADHD), or mental health issues such as low mood or anxiety, along with their families.
Recognising the need for long-term, ongoing support, our Navigators help young people and their families/caregivers to understand their concerns and challenges. They assist in coordinating care and support, accessing information and advice, and connecting with other relevant service and organisations.
This service can be accessed by contacting your GP surgery and asking for a referral.
