Family Support at your GP

 

We offer 1 to 1 support at over 80 GP surgeries across Kent. Our friendly Children’s Health and Wellbeing Navigators help support children, young people (aged up to 18, or 24 for those with SEND), and their families, with a long term health condition, neurodevelopment or sensory disabilities (e.g. ASD and ADHD), or mental health issues such as low mood, anxiety, or lack of social networks.

How we can help

Recognising the need for long-term support, our Navigators help young people and their families/carers, to understand their concerns and challenges and help coordinate care and support, access information and guidance, and link with other relevant services and organisations.
