One-to-one support
There are times where we all need some support, to be listened to, and given time to explore options that will help to improve our personal situation, and our health and wellbeing. This may include aids to remain independent at home, access to benefits, or help to get out and meet new people.
We understand that it’s sometimes difficult to reach out and ask for help. We will work alongside you at your own pace, and when you are ready.
Working together, we will identify what matters to you, what is going well in your life, and where you feel you may need some support. Using the information they gather, our Navigators will create a plan of what your priorities are and what may be available for you to access, either through Involve, or via referral to other support services.
We can support you if you are struggling financially, using food banks, struggling with mental health, or who are socially isolated to address your situation, and signpost you to local services in the community that can provide further support.
We offer practical advice and support for people aged 55 and over, to help you live safely and independently. We can help you build a social network, access health and social care as well as other local services.
We can provide 1 to 1 support in over 100 GP surgeries across Kent and Medway. Helping you access information and support in your community if you are feeling isolated, need support with housing, grants, benefits, or managing a health condition.
