Children’s Health and Wellbeing Navigator – Malling GP Primary Care Network

Join a well-established, progressive charity, providing an innovative and unique service for children and families across Kent.

Working with GP practices to identify and support children, young people and their families through a potential diagnosis pathway eg mental health, neurodevelopmental and other health conditions. Advocating for them and bringing together all professionals in their lives to ensure a coordinated approach to their care and support. Helping them to access information, groups, and inclusive activities that they can enjoy.

We are looking for someone with excellent communication skills who enjoys working with children and families. Experience of working with a holistic and person-centred approach and has strong IT skills with experience of entering personal data accurately. It’s important to be confident in a multi-disciplinary environment, flexible, can-do approach and able to build positive relationships with other professionals as well as support the whole family.
